Community Health Needs Assessment
The 2010 passage of the Patient Protection and Affordable Care Act (PPACA) ushered in new reporting requirements for health care organizations. Per IRC Section 501(r), private, nonprofit hospitals must:
Conduct a community health needs assessment (CHNA) at least once every three years on a facility-by-facility basis.
Identify action plans and strategies to address community needs identified in the assessment and report needs not being addressed (with reasons why such needs are not being addressed).
Report CHNA results to the public.
This community health needs assessment, which describes both a process and a document, is intended to document Polara Healthβs compliance with IRC Section 501(r). Health needs of the community have been identified and prioritized so that Polara Health may adopt an implementation strategy to address specific needs of the community.
Coping With Stress
Stress is hard to define because it means different things to different people; however, itβs clear that most stress is a negative feeling rather than a positive feeling. A well-known tool used by health professionals to measure the amount of stress someoneβs experiencing is the Holmes-Rahe Life Stress Inventory.
You can also utilize the Mental Health America Stress Screener.
